Self-Identity in John Green’s The Fault in Our Stars People with cancer often begin to define themselves based on their experience with their illness, this self-definition through one’s cancer is one that the characters fear in John Green’s The Fault in Our Stars. The novel shows how the characters strive to discover their identities, but despite that are still identified by their illness. The novel also makes the argument that young people with cancer are not any more virtuous or different than other kids rather, they are just normal kids living with an illness. Augustus wants to be remembered and also be more than just a boy who battled cancer, but despite his efforts is still identified by his illness. Augustus also tells her about his family and sisters then asks her â€Å"’So what’s your story?’† she then then replies â€Å"’I already told you my story. I was diagnosed when-‘† He interrupts and says â€Å"’No, not your cancer story. Your story. Despite government efforts to isolateand eliminate its use, it is clear that the use of marijuana is still verypopular. There is an obvious problem concerning marijuana today. Governments on allthree levels: local, state, and federal are trying desperately to find anappropriate policy involving marijuana. National polls show that more than70% of the American people, from both ends of the political spectrum, supportcontrolled access to marijuana for medicinal purposes. Despite fierceopposition from the federal government, voters in California and Arizonapassed ballot in itiatives in the fall of 1996 favoring the legalization ofmedicinal marijuana. If support for marijuana at least as a medicinal remedyis so high, then why have only a few states taken steps to change theirpolicy? There are several reasons why marijuana remains illegal. Mainly, itis a political issue kicked around by certain special interest groups. Someof these groups perceive marijuana as a threat to the home, tearing familiesapart and causing them to abandon traditional values. However these groupsusually are not legitimate areas of legislation. The more powerful groupshave other, more practical reasons for keeping marijuana illegal. Among themost powerful of these groups are the combined lawenforcement-judiciary-penal systems. This group sees the elimination ofmarijuana laws as a threat to their jobs. Add to this group defense lawyers,who stand to make millions of dollars defending marijuana offenders. Consciously or not, they support anti-marijuana laws. Another interest groupincludes the scientists whose marijuana research is funded by the government. If marijuana were legalized, they would lose millions of dollars in researchgrants intended to prove the detrimental effects of the substance. Two otherunrelated but very influential groups are the liquor lobby and pharmaceuticalcompanies. Their spending is usually very secretive and not publicized verymuch. Legalization of a competing product that can be produced with relativeease by anyone with access to a plot of land would cut deeply into theirprofits. And the drug companies want control, rather than just a ban, forthey know the medicinal benefits of marijuana . Therefore the major reasonmarijuana continues to remain illegal, is that special interest groups areblocking legislation by extensive lobbying. Clearly it is seen that manypeople support its use, at least for medical reasons. It is obvious that the current policy for marijuana is not working veryefficiently. The government spends billions of dollars every year to stop itsuse. This leads to the opening of a very extensive black market formarijuana, because the drug is still in high demand. With the black marketcomes all the crime and violent acts that create a new problem ofovercrowding prison populations. In effect, the government does not reallysolve the marijuana problem; instead it just creates a new one in its place. The present policy on marijuana is that it is classified as a Schedule I drugin the Controlled Substances Act. This law established criteria fordetermining which substances should be controlled, mechanisms for reducingthe availability of controlled drugs, and a structure of penalties forillegal distribution and possession of controlled drugs. The criteria forSchedule I substances are: The drug or other substance has a high potentialfor abuse, is not currently accepted for use in medical treatment in theUnited States, has not been proven safe for use under medical supervision. Along with marijuana, hashish, and THC, drugs listed in Schedule 1 areheroin, LSD, mescaline, peyote, and many other hallucinogens. This makes itillegal for anyone to buy, sell, grow, or possess any amount of marijuanaanywhere in the United States. State laws vary in terms of penalties issued. In asthma patients, several studies haveshown that THC acts as a bronchodilator and reserves bronchial constriction. In treating epilepsy, marijuana is used to prevent both grande mal and otherepileptic seizures in some patients. Marijuana also limits the muscle painand spastically caused by multiple sclerosis and it relieves tremor andunsteady gait. Lastly, marijuana has been clinically shown to be effective inrelieving muscle spasm and spasticity. History of Marijuana Laws The hemp plant was once a widely cultivated plant in the New World bysettlers. It has been known for centuries that the fiber from the hemp plantis very useful in making ropes. Therefore the cultivation of the hemp plantwas encouraged and much needed. The Virginia Assembly, urging farmers to growthe crop for its fiber passed the first law concerning the hemp plant in1619. There was virtually no significant legislation passed concerning thehemp plant until the 1900s. It was at this time when American attitudestowards Mexicans became hostile. Marijuana obtained a foul reputation whenMexican peasants crossed the border into Texas. It was widely used by Mexicanpeasants as an intoxicant. The Texas police claimed that marijuana causedthese Mexican settlers to commit violent crimes. Therefore in 1914, the firstban on possession of marijuana was passed in El Paso, Texas. Many otherstates followed Texas, and in 1937, Congress passed the Marijuana Tax Act. This law made the possession of marijuana illegal anywhere in the UnitedStates. During the McCarthy era, the Boggs Acts were passed to definemandatory minimums for the possession of marijuana. Congress moved to an evenstronger position in 1956 by lengthening these mandatory minimum sentences. Anti-marijuana feelings continued to grow, and state laws often imposedstricter penalties than the federal penalties. In the 1960s, however, astrange phenomenon began to occur. For the first time in history, marijuanause began to rise amongst the white middle class. Many mandatory sentenceswere repealed. This was seen in the Comprehensive Drug Abuse Prevention andControl Act of 1970. Most of the states followed the federal government, andthe possession of marijuana was decriminalized. However in the 1980s thegovernment once again changed its mind, with the passage of the Anti-AbuseAct of 1986, which once again imposed mandatory minimum sentences for a widerange of drug offenses. Ms. Porter joined the U.S. Army Reserves in August 2010. There was a verbal agreement between mother and father that Mr. Straub would provide a home for his son during six weeks of Basic Training, at which time Melissa would resume care and custody of their David. During active duty in the Reserves, Ms. Porter suffered a serious training accident and spent six months recovering. She was recently re-settled in Chicago and attempted to arrange the return of her son to her care. Mr. Straub and his new wife refused and filed a paternity action in Illinois, the conclusion of which he was awarded custody of the son. Our client wants to understand wh at rights she has to the return of her son under applicable State and Federal statutes, as well as any other issues or concerns that arise from her service and subsequent injury. Discussion It is likely that the Illinois Appellate Court will overturn the lower court decision on custody jurisdiction and vacate the custody order because it occurred while Ms. Porter was covered by the Service Members Civil Relief Act (50 U.S.C. Â §520, et sec) and the Florida courts had continuing custody jurisdiction under the Uniform Child Custody Jurisdiction and Enforcement Act of 1997 (750 ILCS 36/101 et seq. (West 2004)) (UCCJEA). The original Florida custody order of November 3, 2008 takes precedence over the Illinois custody order obtained in error in 2011. The Uniform Child Custody Jurisdiction and Enforcement Act presents the States with more transparent principles to use in deciding which States have original jurisdiction with regard to paternity and custody determinations. Use Of Abiraterone In Castration Resistant Prostate Cancer Nursing Essay Prostate cancer is the most common type of cancer diagnosis of men in Canada. In 2012, approximately 26,500 men in Canada were diagnosed with prostate cancer and approximately 4,000 died from the disease . Prostate cancer is strongly influenced by androgenic steroids . In advanced stage prostate cancer patients are typically treated using androgen deprivation therapy. . Androgen therapy is not curative and may only improve patients symptoms and may even reduce metastatic lesions . The benefit seen in androgen deprivation is often reduced over years of androgen therapy as the tumour will continue to grow despite low androgen levels. When this occurs the tumour is known to be castration resistant and is known to lead to prostate cancer- related mortality . Castration-resistant prostate cancer (CRPC) may present as either a continuous rise in serum prostate- specific antigen levels (PSA), progression of pre-existing disease, or appearance of new metastases . Many endocrine based therapies have been evaluated for CRPC with minimal success in prolonging patient survival . Systemic nonhormonal therapies using docetaxel, carbaztaxel, and active cellular immunotherapy with sipuleucel-T have been shown to prolong survival . A new drug abiraterone (Zytiga) has shown to improve survival and lower PSA levels in patients with CRPC. This article is to be used by health professionals to gain a thorough understanding of abiraterones indication, mechanism of action, side effects, dosage, clinical evidence, monitoring parameters, and contraindications. Indication According to the product monograph for abiraterone the drug is indicated with prednisone for the treatment of CRPC in patients who have received prior chemotherapy containing docetaxel. Mechanism of Action Abiraterone is formulated as a prodrug, abiraterone acetate, which is converted to abiraterone by the body and acts as a selective inhibitor of 17ÃŽÂ ±-hydroxylase/C17,20-lyase (CYP17) . CYP17 is responsible for androgen biosynthesis in testicular, adrenal and prostatic tumor tissues . As illustrated in Figure 1, CYP17 catalyzes the conversion of pregnolone and progesterone into testosterone precursors, DHEA and androstenedione . Abiraterones mechanism of action in androgen synthesis Side Effects The most common side effects of abiraterone are caused by the mechanism of the drug on CYP17. As seen in Figure 2, inhibition of CYP17 causes on increase in the production of mineralocorticoids, which lead to hypokalemia, fluid retention, and hypertension . Effects of abiraterone on mineralocorticoid production . Dosage and Administration The recommended dosage of abiraterone is 1000 milligrams daily as a single dose . The tablets need to be swallowed whole and the dose should be taken on an empty stomach . To reduce the mineralocorticoid effects of abiraterone the patient should also use a low dose prednisone. The recommended dose or prednisone is 10 mg daily . Clinical Trials Evaluating Abiraterone The effectiveness of abiraterone in CRPC has been demonstrated in several randomized trials. In 2009, a study Attard et al., using forty-two chemotherapy-naÃÆ' ¯ve patients with CRPC received 1000 mg abiraterone . At follow up (median 505 days) a decline in PSA of à ¢Ã¢â‚¬ °Ã‚ ¥ 50% of was observed in 67% of patients, with declines of à ¢Ã¢â‚¬ °Ã‚ ¥ 90% in 19% of patients . A 2010 study by Danila et al., of fifty-eight men with CRPC previously on docetaxel received abiraterone 1000 mg daily with 5 mg of prednisone . Results of study illustrated that 36 % of patients had a PSA decline of à ¢Ã¢â‚¬ °Ã‚ ¥ 50% . In 2011, De Bono et al. conducted a study including 1195 patients who had previously received docetaxel . Groups were assigned in a 2:1 ratio to receive 5 mg of prednisone twice daily with either 1000 mg of abiraterone or placebo . The primary end point of the study was overall survival . The secondary endpoints of the study were time to prostate-specific antigen progression, progression-free survival according to radiological findings based on specified criteria, and PSA response rate . At a median 12.8 month follow-up overall survival was significantly longer in the abiraterone group compared to the placebo (14.8 months vs. 10.9 months, p. The secondary end points all favoured the treatment group; time to PSA progression (10.2 vs. 6.6 months; p. The studies using abiraterone in CRPC illustrate that the drug provides a moderate improvement in cancer progression and improves survival in patients. Monitoring Parameters Health professionals should place on emphasis of monitoring patients using abiraterone. Before treatment hypokalemia and hypertension must be normalized in patients who are scheduled to use the drug . It is important to routinely monitoring a patients blood pressure while they are on this product. Potassium levels should be checked at baseline and regularly to prevent hypokalemia in patients. Patients should also be checked for signs of fluid retention, such as peripheral edema. Liver function tests (ALT, AST, and bilirubin) should be measured prior to initiating abiraterone treatment and continued every two weeks for the first three months of treatment and monthly afterwards . Patients should routinely be assessed for other common side effects, including joint pain and urinary tract infections. Conclusion CRPC is a very advanced stage of prostate cancer that is fatal. Abiraterone (Zytiga) has been shown to prolong survival and lower PSA in men with CRPC. The drug works by selectively inhibiting CYP17 and lowering the production of testosterone via upstream inhibition of precursors in prostate, adrenal, testicular tissues. The selective inhibition of CYP17 leads to a large increase in mineralocorticoid activity in the body leading to hypertension, edema, and hypokalemia. The excess mineralocorticoid activity is typically diminished by administering 10 mg of prednisone daily with abiraterone. It is important to note that abiraterone is indicated as a second line therapy for patients who have failed treatment with docetaxel. While on abiraterone patients need to be monitored for hepatic function, hypertension, hypokalemia, and edema. .

Cause of Cyberbullying Essay

Cyber-bullying is when a person uses the Internet in order to deliberately cause repeated harm to another individual or to embarrass them. Cyber-bullying can include threats, sexual harassment or continued unwanted contact with a person via email, social networks, forums or instant messaging. Cyber-bullies have also been known to post personal information such as phone numbers, addresses and full names on forums or other websites. Almost half of American teens have experienced cyber-bullying. In a study conducted by iSafe. rg, 42 percent of kids have been bullied at least once while online. Twenty-five percent of those kids have had this happen multiple times. One in five kids has been threatened multiple times online. Over 20 percent of kids have received threatening emails. Fifty-eight percent of kids have not told anyone about their cyber-bullying experiences. Almost half of the kids who experienced cyber-bullying had no idea of their perpetrator’s identity. Many individuals choose to cyber-bully others online because they can hide their identity. Cyber-bullies often create fake screen names, social profiles, and email addresses in order to harass and bully their victim without being recognized. This allows the bully to be free of getting into real trouble for their behavior. There can be some serious effects on the victims of cyber-bullying. Many victims have increased suicidal thoughts, fear, worry, frustration, anger, depression, becoming paranoid and a decrease in self-esteem. Sometimes a victim may even begin avoiding their friends and normal activities due to their anger and frustration. Teens often begin to do worse in their classes due to their minds not being on their school work but on their cyber-bully. The effects of cyber-bullying can, in rare cases, cause a victim to commit suicide. There have been four cases of suicides due to cyber-bullying recorded in the United States. Cyber-bullying adult victims can cause the victim to lose earnings, reputation and employment opportunities. Cyber-bullies may post the victim’s name and make up various lies about the person, defaming them. Many employment companies do a web search on a person’s name before hiring them and this may be something that they find. Children and teenagers are becoming more technologically savvy and are looking to the Internet and their cell phones for entertainment, and to social networks to meet new people who share the same interests as them. It was only a matter of time before bullying made its way to the Internet, especially through modes like MySpace and Facebook. Cyber-bullies have been making news lately with the recent deaths of children across the nation who has perished because of online bullying campaigns. Some examples of cyber-bullying include sending threatening emails, text messages or comments to another person; tricking someone into revealing private or embarrassing information and sending it to others; breaking into someone’s email or social media account and sending false messages to others while posing as this person; and creating web pages to make fun of another person. Even though the bullying doesn’t take place on a one-on-one basis, Myspace cyber-bullying and other forms of cyber-bullying still show the same effects. Rather than just being bullied while in the classroom or playground, children can be the target of cyber-bullying 24 hours a day, seven days a week. Some of the more general effects of cyber bullying include poor performance at school, depression and low self-esteem. Because cyber-bullying can take place outside of the classroom and at a more frequent rate, the child may experience more extreme cases of these effects. Cyber-bullying also can happen accidentally. The impersonal nature of text messages, IMs, and emails make it very hard to detect the sender’s tone, one teen’s joke or sense of humor could be another’s devastating insult. Nevertheless, a repeated pattern of emails, text messages, and online posts is rarely accidental. Bullying has been around for many generations and some people would say that it is just a part of growing up. If your child experiences physical threats, report it to the owners of the websites or local officials and keep a log of the bullying. As the saying goes, â€Å"What doesn’t kill you only makes you stronger. † However in today’s technologically advanced age, the death of a child is never worth it.

This sort of conflicting upbringing is apparent in the novel, The Adventures of Huckleberry Finn. The main character, Huck, originally raised by his father, later goes to live with the Widow Douglas, who tries to sivilize Huck. Also in the house lives Miss Watson who owns a slave, Jim. Between Pap, the Widow and Miss Watson, Huck becomes aware of many different ideas and beliefs, especially about slavery. When Huck escapes Pap, he runs into Jim on Jackson Island, where Huck is faced with a moral dilemma. Jim reveals to Huck that he has runaway, but Huck has promised, not to tell. Huck agrees to help Jim get to freedom. He sometimes feels that what he is doing is wrong, but other times he looks beyond the fact that Jim is a n***er and sees him as his friend. Throughout the novel Huckleberry Finn, Huck faces an internal confl ict with his conscience on the topic of slavery. In the beginning of the novel, Huck goes back and forth between sivilized life with the Widow and Miss Watson, and life with Pap. Hucks sivilized side is that which condemns him for helping Jim to escape, while his life with Pap, which does not directly reflect Paps views, yet the fact that Pap does not instill beliefs in Huck that are accepted by society. Hucks wanting to help Jim goes against what is accepted by society. This situation lays the foundation for Hucks internal conflict with his conscience about slavery. Since Miss Watson owns Jim, Huck views him as her property, and Huck faces a dilemma when Jim reveals he has run away. Acne is a skin condition that can cause pimples such as, white heads and black heads to appear on the face, chest, back, neck, and shoulders. Most teenagers have acne for about five years, but for others it clears up much faster. Women can also get minor acne before there menstrual period. All of us are affected by acne at one point in time or another, and some less than others, but all of us experience it and just have to deal with it because the best remedy for getting rid of acne is time. Acne begins in the upper part of the hair follicle, where sebaceous glands discharge sebum, an oily substance that keeps skin from drying out. During puberty, the sebaceous glands enlarge dramatically. If pores near these glands become clogged with dead skin cells or oily cosmetics, the sebum accumulates underneath, causing inflammation in the surrounding skin. The acne is further aggravated when bacterial enzymes break down the sebum into irritating substances that add to the inflammation and swelling. If several follicles in the same area become inflamed, scarring can also occur. There are several forms of acne, but the most common is vulgaris, found mostly in teenagers, whose hormonal changes cause a large increase in the size of the sebaceous glands and in sebum production. As stated before the best remedy for acne is time, and just letting acne go away on its own is the best. People with moderate cases of acne may use topical antibiotics, and also take small doses of antibiotics given orally over long periods of time. Other therapies that have been found effective include the topical use of retinoic acid or benzoyl peroxide. This report describes the dynamics of the workplace that both hinder and support training in HRD, mitigating issues of organisational culture, and the potential conflict that can arise between theory and tangible HR practice when attempting to build human capital. Why training is an imperative Sainsbury’s positions itself on the market as a value leader and as an organisation with a strict compliance to multiple dimensions of corporate social responsibility to maintain a competitive edge (Sainsbury 2011). At the highest levels of governance, with decision-making occurring vertically throughout the organisational hierarchy, Sainsbury leadership establishes an ethical climate built on integrity, transparency and trustworthiness which are then disseminated throughout the organisational culture. Establishment of an ethical climate is quite different from development of an organisational culture, defined as the methodology by which employees perceive the established norms of the business culture (Denison 1996; Bartels et al. 1998). The premise of this ethical climate and supporting ethical culture is to ensure that the values and principles of Sainsbury’s business model are modelled by employees and managers to improve Sainsbury’s market reputation with mu ltiple stakeholders and shareholders. Why is this important in the domain of HRD? Sainsbury, in order to maintain its high market share in this saturated marketplace, must ensure that the ethical and socially-responsible values are transparent and adopted throughout the organisational model. Sainsbury differentiates itself from major competitors such as Tesco and Morrison’s through branding, â€Å"a core marketing practice emphasising the continuity of the firm with important buyer markets†, translating the intangible of market-based assets to a tangible representation of value (Abimbola 2001, p.98). Sainsbury establishes a brand personality in dimensions of sincerity, competence, and sophistication, three dimensions necessary to gain market loyalty and subsequent brand equity (Aaker 1996). However, in order to provide tangible and recognisable value associated with this established brand personality, employees must be properly developed so that job role functions are aligned with core values and the elements of brand that leads to competitive advantages. This cannot be effectively accomplished without establishing a training programme that is homogenous and relevant for issues of ethics and social responsibility.
